Thank you for choosing to train with Severn Hospice.
Whether you are seeking additional qualifications or refreshing your training, use the booking form below to reserve your place on a course in our prospectus. We will confirm your place when we issue your invoice.
Any payments help fund both the training and our caring services.

Please note that all courses start promptly.
If you are running late or are stuck in traffic please call us on 01743 236565 to let the course leader know. Late entry once the training day has started will not be possible.
To book on to a course, please complete the form ensuring that you select your answers from the drop down options and that all boxes are filled.
If you have any special requirements that may affect your ability to vacate the building quickly in the event of an emergency, please contact Gail Stewart on 01952 221351 or via email gails@severnhospice.org.uk. She will talk to you about any additional measures we may need to put in place.
How to pay your invoice
When you submit this form, we will reserve your place/s and send you an invoice and there are two ways to pay this, by cheque or BACS.
To pay by BACS, you will need the invoice number we send you, quote ref SJO and your company name, and send your payment to:

If you prefer to pay by cheque, please make it payable to ‘Severn Hospice’ and write the invoice number on the back, and post to Sue Jones, Education Secretary, Bicton Heath, Shrewsbury, Shropshire SY3 8HS.
We will reserve your place and confirm the booking on receipt of payment.
If you change your mind, you can cancel without charge up to seven days before your course date. Places cancelled within seven days of the course will be charged at the full rate, unless there are exceptional circumstances.
