Your Right to See the Information We Have About You

Under data protection laws (GDPR), you have the right to know what personal information we hold about you. If you’d like to see this information, you can make what’s called a Subject Access Request (SAR). We’ll then provide details about what data we have, how we use it, and why we have it. We’ll also give you a copy of the information in an easy-to-understand format. Usually, we’ll respond within a month, unless something unusual comes up that might take a little longer.

It’s easy to make a request — Please use this form to let us know what information you’re looking for, and we’ll take care of the rest. To keep your data safe, we will need to confirm your identity before sharing anything.

Your Right to Correct Information

If you notice that any of the personal information we have about you is wrong or incomplete, you can ask us to fix it. It’s important to us that your data is accurate and up-to-date, so we’ll make the change as soon as possible once you let us know.

Please use this form to let us know what needs to be changed and provide any details we might need to make the correction. Like with access requests, we might ask for some proof of identity to ensure your data stays secure.

We want to make this as easy as possible for you, so please don’t hesitate to get in touch if you have any questions or need support. The easiest way to do this is to send us an email via information@severnhospice.org.uk and one of the team will contact you back as soon as we can.
