Hospice at Home

We are able to care for four out of five people in their own homes.

Our community nurses and healthcare assistants are palliative care specialists who work with the whole healthcare network supporting you, including your hospital, GP and District Nurse.

The team covers people who live in Shropshire, Telford & Wrekin and North Powys and will support you and your family and carers.

What to expect

Once referred to our care, our nurses will make an assessment of your needs to ensure your care is individually tailored and personal. Our care will complement but not replace the care you receive from other healthcare professionals.

The team has extensive skills, training and experience so is able to care for someone’s whole well-being, not just their clinical needs.

They will do this both in person and over the phone, matching their care with the person’s needs at that time and co-ordinating with the other professionals and agencies supporting them.

How much support is needed from our nurses, and from our supportive therapy services, will be decided in partnership with the person, their family and all those involved in their care.

It can include care from our nurse practitioners, night and day care, or support in our ‘virtual hospice’ – consultant-led care which allows you to stay at home while still being closely monitored every day.

The specialist nursing care and advice they provide will enable you to live as well as you can and have the best quality of life possible within the constraints of your illness.

We will always prioritise our care for patients with the greatest need.

We are happy to answer any questions you may have to help reduce the stress that can be felt during what we understand is a difficult and worrying time.
