Other Help at Home
When you are referred to Severn Hospice we may be able to provide some services to you in your own home.
The main services we can provide at home are:
Outreach Team
We manage a team of clinical nurse specialists who may provide help and advice to you if you have been referred to the Hospice. They are experienced in the issues that complex conditions can create and will be able to work with you and your carers to help you tackle these.
The outreach team can visit you in your home, see you when you visit one of our Day Hospices or one of our clinics, or they can talk to you on the phone.
You can be referred by your GP directly to the outreach team, or the outreach team might become involved in your care after you have been referred to another service within the Hospice.
Occupational Therapy
We want to help you to improve the quality of your life and to make sure you remain in control of the decisions made about your care.
We employ occupational therapists who can help you to cope with your changing abilities. They are specialists in helping you to remain independent while making sure you are comfortable and safe.
We can provide simple changes, like legs to raise your furniture and to make it easy for you to get in and out of your sofa on your own. We can also help you to get hold of a huge range of specialist aids, furniture and changes to your home to make you as independent as possible.
As well as equipment and physical changes we can work with you to look at things like lifestyle management (identifying priorities, making choices and adapting to change), self esteem and confidence issue, fatigue and breathlessness management, anxiety and safety.
