Hospice at Home
Hospice at Home is a service provided by Severn Hospice along with Marie Curie and with the support of Shropshire and Telford and Wrekin Primary Care Trusts.
Why you would receive support from Hospice at Home
We can provide care in your own home if you are in the final six months of your life. We can offer support if you are experiencing an acute episode of your illness or if you are very ill but want to remain at home at the end of your life.
Hospice at Home is available to you if you live in Shropshire or Telford and Wrekin. At the moment the service is not funded in Montgomeryshire, so unfortunately we are not currently able to offer this support there. We are working hard to bring Hospice at Home to mid Wales.
How referral works
You must be referred to us by your community nurse or a member of our outreach team. They will get your permission before making a referral. When you are referred to us a member of our Hospice at Home team will contact you and arrange to visit you. They will discuss your particular needs with you, and then arrange for a registered nurse or a healthcare assistant to visit you and provide care for you in your home.
We deploy Marie Curie nurses as well as our own staff. By working together like this we can make sure that you receive the support that you need from the most appropriate healthcare professional.
The care we provide
Hospice at Home provides services which are tailored to your particular circumstances. We provide palliative care, which means addressing not just your medical needs but also your spiritual and practical concerns.
We provide the very best nursing and social care and work to improve the quality of your life. We want to make sure that you have a dignified and peaceful death. As well as providing care directly to you, we can offer support to your relatives and carers. We give appropriate and accurate information at all times and try to reduce the stress that can be felt by your relatives and carers.
A nurse or healthcare assistant will visit you at agreed times and spend time with you. This can be during the day or at night, and for different lengths of time. All of these decisions will be agreed with you when you are referred to our services
We work closely with the other healthcare professionals providing care to you (people like your GP or your community nurse). We don’t replace them, we complement them.
Other things it might be useful to know
You will not need to pay for this service. Hospice at Home is funded and supported by Severn Hospice, Marie Curie, Shropshire County Primary Care Trust and Telford and Wrekin Primary Care Trust.
You can help the nurse who is visiting your home by making sure they have clear access around your house, a comfortable chair to sit in when not attending to your care needs, the use of a telephone (so they can tell us they have arrived safely), and somewhere to make tea, coffee or another drink while they are with you.
Please let us know if you have any pets or hazards in your home.
Please make sure that no-one smokes while the nurse is there or for at least an hour before they arrive.
