Referrals
Referring patients to Severn Hospice
You must complete a referral form in order to refer a patient to our services. You can download a copy of the referral form here. Patients must be referred by their General Practitioner or Hospital Consultant. Other professionals can refer patients with the consent of their General Practitioner.
Admission will be based solely on the basis of need and the ability of the Hospice staff to meet those needs. Patients with the following conditions will be considered appropriate for assessment:
- advanced cancer – including those without symptom control needs but with major psychosocial needs
- end stage respiratory/renal/liver failure
- end stage cardiac failure.
- progressive illnesses such as Motor Neurone Disease and Multiple Sclerosis
Patients will be considered for admission for symptom control, including psychosocial need in the absence of physical symptoms and terminal care.
Admissions meetings take place on Monday, Wednesday and Friday mornings.
We are not an acute service and cannot accept emergency admissions, although we can arrange to accept out of hours admissions.
