Healthcare Professionals
Severn Hospice is part of the healthcare service offered to people living in Shropshire, Telford & Wrekin and North Powys but we are not part of the NHS. We are an independent charity funded through a combination of the kind donations from local people, fundraising activities, our Lottery and retail departments, gifts in people’s wills and grants from the NHS.
The services we provide are free of charge for our patients and their loved ones. We care for people living within our catchment area (link to map) but can sometimes accept patients from out of the area if it is appropriate.
The services we provide:
Referring a patient
A patient can be referred to any of our services by their GP, Hospital Consultant or any other healthcare professional, with the consent of the patient and their GP.
Referrals will be assessed on the needs of the patient, the ability of the hospice to meet those needs and service availability.
Criteria
Patients who have been diagnosed with a life limiting illness can be referred to the hospice
To make a referral please download and complete in full our referral form.
**Please note that all parts of the form must be completed, incomplete forms will be returned and will result in delay for the patient.**
The reason for referral must be clearly stated on the referral form which should be faxed or posted to the hospice. The form should be accompanied by supporting documentation including hospital letters, reports or GP summary and a list of current medication.
The hospice is not an emergency service however we can accept out of hours or emergency admissions, if the hospice has the capacity to admit the patient. In the case of an out of hours or emergency admission please fax over the completed referral form and follow up with a phone call. All other referrals will be contacted as soon as possible.
