Severn Hospice launches new community service

Hospice at Home logoWe are delighted to announce the merger of all our existing community teams into one service, called Severn Hospice at Home. This move will enhance the support offered to terminally ill patients who want to be treated in their own home by improving access and broadening the range of illnesses our community staff care for.

Severn Hospice at Home will join together all of our community based teams, including our group of Clinical Nurse Specialists which was recently boosted by the transfer of all the region’s former Macmillan community nurses to the team.

Other support provided by the service will include nursing care overnight and during the day, provision of helpful equipment, specialist advice and management of symptoms, help with day-to-day activities and emotional and psychological support. Severn Hospice has provided specialist support to patients in the community through various teams for a number of years and the time is now right to officially join them together.

Heather Palin, Nursing Director, said ‘Severn Hospice at Home marks an exciting new chapter both for us and for our patients. It will improve access to our community services for patients with a life-threatening illness and will allow us for the first time to move towards caring for patients with a much wider spectrum of conditions.

‘We understand that for some patients, having the choice to remain in their own home whilst receiving treatment for their illness or when they are nearing the end of their life is very important. This is why we have developed an integrated team that offers a comprehensive range of services which we can tailor to suit each individual patient’s needs.

‘By formally merging all of our community staff into a single, unified team we are creating an even better service for our patients and fellow healthcare professionals by extending our Hospice’s special kind of care beyond the walls of our three sites in Shrewsbury, Telford and Newtown and into communities across the region – effectively creating a ‘Severn Hospice on your street’.’
